Name spelling on the application MUST exactly match the passport machine-readable zone — mismatch is the #1 rejection cause. Date of birth and address on form, NID, and utility bill must all match exactly. No information about appointment slots should be purchased from agents/intermediaries — IVAC slots are FREE; any payment demand is fraud. IVAC locations: Dhaka (Jamuna Future Park), Chittagong, Rajshahi, Sylhet, Khulna. Long-stay categories (student, medical, business 6m+) may require FRRO registration within 14 days of India arrival.
